- Open the HealthHub sign-in page.
- Enter your email and password, then press Sign in.
- Enter the six-digit code sent to your email and press Verify.
- Open Active Care Patients from the left menu.

- Mark pre-triage as reviewed.
- Press Choose starting stage.

Enroll a patient and assign a navigator

- As a head navigator, choose a navigator for an unassigned patient from the list.
- To enroll a patient, press Add care patient.
- Find the patient, choose the care program and add them. This sends an invitation.
- To transfer responsibility, open the patient record and choose Hand over.

- Check the stage, step, task owner, due date and completion status.
- Check for tasks marked Hidden from the patient.

- Find the hidden task.
- Press Show to [patient], then Yes, show it.
- To hide it again, open the task menu and choose Hide from [patient].
- When changing status, select only the tasks the patient should see now.

- Enter the relevant results and care details.

Add a task and set its details

- Press Add a task to this step.
- Enter a short name and choose the task type.
- Choose who completes it and set its due timing.
- Add the patient-facing description.
- Press Add task. The task starts hidden.
- Show the task to the patient when ready; its due date starts from that day.

Steps
- Tick the task you completed.
- Press Yes, mark done.
- To undo, open the task menu and choose Mark as undone.
- For a patient care task, use Mark done for [patient] only after confirming it was completed.

- Press Change status.
- Select the next stage and press Save status.
- Select the tasks the patient should see.
- Confirm and save.

- Complete the current step's tasks.
- Press Change status.
- Choose the next step and save.
- Select the tasks to show in HealthPal.

- Enter the task name, type and due timing.

Find patients who need follow-up

Steps
- Open Active Care Patients → List → Filters.
- Set Next-step status to Overdue to find patients past their stage target.
- Press Clear all.
- Set Risk state to Needs follow-up and choose a flag if needed.
- Open each patient to review what needs attention. These filters are not individual-task overdue filters.

Book an appointment for a patient

- Open the patient's Appointments tab.
- Under Bookings, press Book service.
- Choose the service and review its fee.
- Choose a date and available time.
- Press Book appointment.
- Check the booking appears. The patient is notified in HealthPal.

- Open the patient's Appointments tab.
- Review the date, service, time and status of each booking.
- Check today's appointment pinned at the top of the patient's page.

Reschedule or cancel an appointment

- Open Appointments and press Update on the booking.
- To reschedule, choose Change the date or time.
- Choose an available slot, press Save new time, then Yes, reschedule.
- To cancel instead, choose Cancel the booking and enter a reason.
- Press Cancel booking, then Yes, cancel booking. The patient is notified.

- Open today's pinned appointment and press Update status.
- Choose Patient has arrived, or Patient didn't come as appropriate.
- Save and confirm the change.
- After the visit, choose Their visit is done.
- Save and confirm, then check the completed booking in Appointments.

- On the HealthHub appointment page, scroll to Appointment Notes.
- Press Add New Note.
- Enter a title and remarks.

Navigator: book or decline an appointment request

Steps
- Open the patient's pinned appointment request.
- Press Book service.
- Choose the service, date and available time, then Book appointment.
- If the request cannot be booked, press Decline instead.
- Enter a reason and explain the next action. The patient can submit another request.

Handle callback requests

- Open Callbacks.
- Press Confirm call schedule and set the time.
- After attempting the call, press Mark as called.
- Choose No answer if the patient did not answer; the request stays open.

- Use a clear document name that describes the record, not its findings.
- Press Add record. The patient can view it immediately and is notified.

- Open the document's menu in Health records.
- Choose Attach to task.
- Select a result-viewing task on a step the patient has reached.
- Press Save.

- Before uploading, confirm the document is ready for the patient to see.
- Add the record. Care-team uploads are shared immediately; there is no hide setting.
- Review the file in Health records.

- Tap Upload on the task → Upload New File.
- Take a photo, attach a file or choose from your gallery.
- Check the file name and open Details.

- For a separate upload, open Health Files → Upload File. It starts as Only me; select My provider if you want to share it.

- Tap + → Log → Symptoms.
- Choose the symptom and complete required details. Pain requires a score and location.
- Tap Save symptom log.
- For blood pressure, open Log → Blood pressure and enter both numbers.
- Review the displayed guidance, then Save reading → Done.
- Check today's Logs for the saved entry.

- Tap + → Manage medication → Create a reminder → Add medicine.
- Enter the medicine name and strength from the label.
- Confirm the amount, schedule, first dose time and duration.
- Tap Save medicine.
- After taking a dose, tap Log, check the time and Save log.
- For a missed dose, select Mark as missed dose and save. Review the guidance shown.

- Open the menu → Care Team.
- If prompted, tap Explore my care team.
- Tap Add member and choose Caregiver.
- Enter the person's name, contact number and email.
- Check the details, then tap Add member. This sends the invitation immediately.
